Please note that your loved one needs to be able to make decisions and understand these forms for him or herself and write their name for POA documents to be validated by a Public Notary. If your loved one is unable to make his or her own decisions, understand what these documents are for, or write their name, seeking Legal Guardianship for their care is more appropriate.

If you are unable to print the Ohio Advance Directives (The Living Will and Medical POA), these are usually readily available upon request at all hospitals and primary care physician's offices.

OHIO ADVANCE DIRECTIVES

Requires two witnesses or a Notary Public to witness signing

In the event that your loved one is incapacitated, this form allows you to make decisions for the best care for their health, as well as allowing their wishes to be followed by their medical team.
This link includes both the Medical POA and Living Will forms.

GENERAL FINANCIAL POA

Requires a Public Notary to witness signing

The General Financial Power of Attorney permits financial decisions for your loved one up to the point of incapacitation.

DURABLE FINANCIAL POA

Requires a Public Notary to witness signing

The Durable Power of Attorney permits financial decisions for your loved up to and beyond the point of incapacitation.

OHIO LIMITED POA

Requires a Public Notary to witness signing

This Power of Attorney form outlines a specific objective that your loved one wants and/or needs you to do for them.

OHIO MOTOR VEHICLE POA

Requires a Public Notary to witness signing

Most of us own a car. This form allows you to make decisions for your loved one's car on their behalf.

OHIO REAL ESTATE POA

Requires a Public Notary to witness signing

This document allows your loved one to grant you permission to handle and make decisions regarding their home and properties.

OHIO LIVING WILL

Requires two witnesses or a Notary Public to witness signing

This form provides guidelines for your loved one's medical team to follow in the event that they become near end of life.

OHIO MEDICAL POA

Requires two witnesses or a Public Notary to witness signing

This form allows your loved one to choose someone to represent them and make medical decisions on their behalf.

OHIO REVOCATION OF POA

Requires a Public Notary to witness signing

If your loved one is unhappy with how their chosen POA is handling their care and the decisions necessary for their best interest, this form allows your loved one to revoke their permission of your duties as their POA.
