Please note that your loved one needs to be able to make decisions and understand these forms for him or herself and write their name for POA documents to be validated by a Public Notary. If your loved one is unable to make his or her own decisions, understand what these documents are for, or write their name, seeking Legal Guardianship for their care is more appropriate.

If you are unable to print the Ohio Advance Directives (The Living Will and Medical POA), these are usually readily available upon request at all hospitals and primary care physician's offices.

Requires two witnesses or a Notary Public to witness signing
In the event that your loved one is incapacitated, this form allows you to make decisions for the best care for their health, as well as allowing their wishes to be followed by their medical team.
